If you choose to continue your treatment with Fort Worth Plastic Surgery, the remaining physicians and staff are ready and willing to manage your ongoing treatment and follow-up care.

Your records can be obtained and transferred upon your written authorization, in accordance with Texas law and subject to reasonable, capped (by the Texas Medical Board) copying or administrative fees. You can initiate this transfer by contacting Fort Worth Plastic Surgery at (817) 810-0770 to request release of your medical records. If you plan to see any physician outside of Fort Worth Plastic Surgery, obtaining a copy of your recent records and bringing them to your appointment will help ensure a seamless transition.
